Do you have to be 18 to buy medicines ( OTC) from walmart?

Not cough medicines, more a long the lines of laxative.

Not at all. So long as the drug does not contain pseudoephedrine, it is perfectly legal to purchase. IE: Tylenol, vitamins, Excedrin, and yes, laxatives.
