How do i inform the doctors surgery that ive legally changed my name?

In the UK you can, for most purposes, change your name without any formalities as long as it is done without intent to deceive, defraud etc.
Ring or visit yor surgery with the new information - they may request the change in writing with your old and new signatures but it reall is no big deal. If you changed your name on marriage this would be very similar to achieve. If you have changed your name by deed poll or another legal instruement you could give a copy to your doctor and to all other interested parties.
